I ran into 1 tied in 2 pipe. It didnt work, The return line shut off the air vent before it got hot. It was on the 1st floor with an unfinished basement so i put a 30 inch deep trap on the return with a drain valve on the bottom. it worked a lot better but not perfect.

Effical makes a jumper to bypass damper, they are sold with larger steam boilers that dont come with a vent damper.

I believe a thermaflo is the same as a union steam or williamson basicly a weil mc lain generic. If that is the case a 150 and 125 btu have the same block so you can change burner tray to a 125 000 tray and lower to 325 sq ft of steam. also when you add a baseboard loop off boiler you do not add it to size of boiler you only use  boilers pickup factor.

Do you have 2 pressure controls? If so one is a high limit and will turn the pilot out.

I dont know about 3/8but they do make 1/4 in. I think markal makes it, its been a while since i had some. Now that you made me think about it i think i will order a few rolls.

I see a problem, They tied the steam return and the condesate return above the water line they should be tied in below the water line.

I read your 1st post again and it seems like it all started after the prv and expantion tank was installed. I would remove them. and without expantion tank you have to get rid of the check valve to.

If incoming water pressure is only 78 lbs why not loose the prv. I dont know your codes but maybe loose check valve and expantion tank, it sounds like thats the way you were before the work done.

Is noise constant or only when running water?  I saw a few jobs that banged when water was running, In those cases it was the reducing valve, it would reduce then not then reduce over and over causing the pipes to bang.

If it says to plug unused 2 1/2 steam tapping then it is 2 in because you can clearly see they used a bushing at tapping.

How is the water level in the sight glass when the boiler is running? If it is boucing allot you need to skim the boiler. I fixed a panting radiator last year, the water level was bouncing and rad was panting. After skimming it stopped.

What size boiler is it? I would be more concerned about the bull head tee on the back main, and that they put the boiler in sideways, The controls are facing the wall making it harder to service and even check the water level.

A drop header wouldnt work there, He said there is 16 in of a dimention. It needs to be in a pit or needs a condesate pump.

Hap, I installed at least 100 boilers including some drop headers. I just dont get how drop headers are easier. You have more pipe more fittings. I cut every boiler out completly including sections of mains. When i put in a drop header i just install more pipe more fittings more labor.

After reading your last post do not return indirect piping into skim tapping or relief tapping, they are above the water level. All boilers have return tappings on both sides supply from one return from the other.

Cut it all out and pipe at boiler specs. Also cut out that bull nose tee. You must use 2 risers off the header. you could also go better and use drop header but at more cost.
